The Autism Society of Greater Akron is not able to provide medical or psychotherapeutic advice, diagnosis, or treatment.

PHOTO RELEASE
I understand that my participant may be photographed and/or videotaped for print or electronic media by the Autism Society of Greater Akron. These are used for training, informational, and celebretory purposes. The identity of participants will not be published without prior permission.
RELEASE OF LIABILITY
I understand that my son/daughter is voluntarily participating in the ASGA LEGO Club. I understand that the meetings are not intended as therapy or treatment for any mental health or physical disorder.
In addition, my son/daughter and I have read the guidelines for the LEGO Club and agree to abide by them.
