Building Brighter Futures: Tools to Envision a Good Life for Your Loved One

During transition planning, all families think about what the future may bring for their child. Parents are expected to work with their child and others who support them to build a good life. When your child has complex behavior or medical needs, this may be a challenge. We have designed a booster session especially for families who face these challenges. Families will be supported and will engage in meaningful conversation to work toward a vision for their child’s and their family’s future.

• Use the Integrated Supports Star to create a discussion of ideas to accomplish a goal or to resolve problems
